Reporting an Incident
If you witness behavior that is concerning or goes against our community values, please report the conduct using the following referral forms. If you are not sure which form is most appropriate, contact Carl Crowe, Dean of Students, in the Office of Student Affairs at 301-687-4312.
PLEASE NOTE: All reports will be evaluated for violations of university policy and criminal law. HOWEVER, this form is NOT a police report; if you are in immediate danger, call University Police at 301-687-4222.
Report a Student Conduct Incident
Complete this form to report student conduct issues including, but not limited to:
- academic dishonesty,
- disorderly conduct,
- alcohol and other drugs,
- harassment, and
- vandalism.

Report Sexual Harassment and Misconduct
Use this link to report incidents of
- sexual misconduct,
- relationship violence,
- stalking, or
- other related misconduct.
You may file this report anonymously by NOT logging in with your FSU credentials and by stating your name as "Anonymous."
Responsible Employees are obligated to report such misconduct to the Title IX Coordinator; such reports can be made via the link above.

Use this link to report an athletic grievance. Grievance issues include, but are not limited to
- Unsafe coaching or playing environment in the athletics department and/or in athletic activities, including team practices, competitions, travel or events;
- Negligent treatment of injuries;
- Bullying and/or hazing activity by coaching staff and/or students;
- Discrimination and/or harassment;
- Violations of other University policies during the course of athletic activities, including, but not limited to the University’s Alcohol Policy or Student-Athlete Code of Conduct;
- Inadequate or improperly maintained equipment;
- Unsafe transportation

Report a General Academic Complaint
Use this link to report complaints related to academic grievances that are not covered by existing policies and complaint procedures. Academic concerns related to online education, with the exception of grade grievance, can be submitted using this form.
